2011 INCOME TAX FORM INFORMATION

The Internal Revenue Service will be sending a limited supply of tax forms to public libraries this tax season. The Massachusetts Department of Revenue will NOT be sending printed tax forms or instructions to the library for distribution. The library will only have a reproducible copy of the Massachusetts Form 1 with instructions available at the Reference Desk for photocopying.

Forms & publications can be printed from the IRS and MA DOR websites.

Federal Forms

www.irs.gov - print out federal tax forms and instructions from the Internal Revenue Service website.
 1-800-829-3676 - call the Internal Revenue Service directly to request federal forms.
 Library Reference Department - photocopy forms from a binder provided by the Internal Revenue Service. Photocopies are $.10/page. (Binder is expected to be available in early February.)

State Forms

 1-800-392-6089 - Call the Department of Revenue directly to request state forms.
 Library Reference Department - A reproducible copy will be available for photocopying.

Tax Preparation Assistance
There will be free tax preparation assistance provided at the Plymouth Council on Aging (not at the library.) The program is open to anyone (not just the elderly) and it is free. However, they do restrict the types of returns they will complete; for instance, taxpayers with rental income, or who day-trade in the stock market, or run a business with more than 1 employee are ineligible. To make an appointment for this assistance or for additional information please contact the Council on Aging at 508-830-4230 AFTER February 1st. They will start preparing returns on 2/15th and continue through 4/13th.

Free income tax preparation for low to moderate taxpayers is available through the South Shore Community Action Council. You are eligible to have your taxes prepared by a trained volunteer if you earn approximately $40,000 or less. Call 508-747-7575 ext. 257 for an appointment.
